Assuming it still works, that's a "mcp73vt-pm", which most sources on the net say it is the same as ECS GF7050VT-M5, just with a different form factor (stuff is rearranged to make one or the other fit a smaller case).
All sources point to the page 24 of this manual
, which is for the ECS GF7050VT-M5, but the pin map for the case stuff should be exactly the same.
Since your cables are clearly labeled from the pics I see and now you have a pin map, it should be a breeze to set it up correctly.
Besides, if you really want to do it the really safe way, it's not that hard to guess from the position of the cables in the old connector.
