So this leads us to believe its a faulty driver

so this seems to be gfx/directx related although the file is question should be updated automatically by win update so its hard to believe its at fault, i would check to ensure your windows is up to date manually and if necessary update gfx drivers manually

also noticed you have a very old driver in your system
