Correct about Never Combine. The open window icons will look like this when hovering on them:

Sorry, I have no answer for you.
With my setting at Never Combine, opening IE, then Notepad, then another IE, I do show another icon not combined.

For Firefox, click on Firefox / Options / Options.
On the Tabs page, uncheck Show tab previews in Windows Taskbar.

Ditto for IE. Tools / Internet Options. Click Settings button under Tabs.
Uncheck Show previews for individual tabs in taskbar*

Color me confused, based on the Start Menu button in the image you posted, simonsez9, it appears to be Windows XP and an old version of Firefox that you are working with. Thus, there will be no option as suggested by Ztrucker on that machine. As I recall, there was an extension I used on older versions of Firefox that prevented the grouping you're referring to but I am not recalling what it is at the moment.
