What's wrong with my graphics card?

    Hey everyone,

I was sent here so here it goes.My graphics card has been really mean to me for the last couple of days.Everytime I start a game, the pc freezes, suddenly dots take over all of my screen, and then BSOD eventually happens.I freshly formatted my pc,got the latest nvidia drivers for my 9800gt,and I just can't figure what's going on.

I'm using windows 7-64 bit, full retail version and my system is almost 1.5 years old.
I attached the files as asked down below.
    Thanks in advance,
      My Computer

Go easy on me please,I'm kind of new to this kind of stuff.
I ran the benchmark test with furmark, however after an hour, the temperature does not exceed 76 degrees celcius, and the average fps is 31.I'm not sure if I should wait till the crash happens or whatnot though.
