It appears that your video card drivers are corrupted. I would uninstall your card from device manager and select the option to remove the drivers. Reboot the computer and then install the latest drivers from the ati site.

    Code:
    Rt64win7.sys Thu Feb 26 04:04:13 2009 (49A65B0D)
Your Realtek drivers are still not updated

I salute Mike for trying to help with so little to go on, apart from Griffs' stuff.

I think your problem is a memory leak caused by third party software. There are various ways to check this but if up to me I'd start simply by turning on TaskMabager, show all Users and sorting the Process screen on max mem usage on top. It will take some time but IF you have a mem leak, the culprit will show itself by the fact it's memory usage keeps growing...maybe slowly... but even as you do nothing.

Why do I think this? You've got about 10 Memory leakage warnings in your error reporting. Am I sure this is the real problem? No... One can have memory leakage and not even notice it... up to the point where an illegal memory access is tried..then you notice.
