BSOD From playing Dragon Age Ultimate Edition, quickly spread

Originally I only got the BSOD while playing Dragon Age Ultimate Edition, but then it started to happen when I played any game. I uninstalled DA:UE and the problem continues, so I reformated my computer, and the problem worsened. I believe my current BSODs are from bad drivers.

USEABLE MEMORY: 8.00 GB

SOUND CARD: (1) High Definition Audio Device (2) High Definition Audio Device
GRAPHICS CARD: ATI Radeon HD 5700 Series
OS: Microsoft Windows 7 Home Premium 64-bit 7601 Multiprocessor Free Service Pack 1

APC_INDEX_MISMATCH
USB_DRIVER ERROR
