I see possible hardware incompatibility. To be sure, please provide more hardware information:
Download and install CPU-Z and Upload screenshots of the CPU, Mainboard, Memory, and SPD tabs. In the SPD tab, upload an image of each slot.

(For your information, the CPU and Motherboard were fine until I got new parts few months ago, the issue started few weeks after.)

I'll have to read through your motherboard manual tomorrow and see if I can help. Can you post a screenshot of each slot under the SPD tab?

I only see one there, and I get the feeling you have mix and match RAM with 2x2 GB and 2x4 GB. Is that correct?

Oh my bad, I did not see those slots :/
Yes I do have 2x2 + 2x4. Let me know if i can provide you with anything else.
p.s. I've already tried switching ram and using only 2x4 or 2x2 due to different voltage, it still didn't work...

What voltage are you running at? The 2x2GB are likely to be more compatible with your processor, although none of the four modules are truly compatible. You have an AM3 processor which will not get along well with XMP modules. You really should be using PC3-10600 1333 non-XMP RAM for the best compatibility.
AMD Phenom - The Phenom II and Athlon II X4 belong to the same chipset, so the same rules apply for RAM.

I'm just running on default voltage but what I can say right now is that I don't believe ram and cpu are causing the blue screen problem because I've had no problem before with same cpu along with just 2x2, and just by using 2x2 which i've tested a month ago, it still causes BSOD.
I'm also planning to get a new CPU+Motherboard in the near future, But I'm not sure if that would solve the problem. Additionally, if I end the process of Java on task management, it seems like BSOD doesn't show/or show often. (I'm not sure if its coincidence or not.)
It also seems like when I play Heroes of Newerth, the chance of BSOD appearing increases to more than 5 times.
*edit* nevermind, its just the Heroes of Newerth that causing it more often, Java doesnt have anything to do with it.

Could this be a bottleneck issue even though there should be enough power? Because this issue is keep happening.. Once again I have Radeon HD6950 and Seasonic AX-750 along with other hardwares.
