BSOD after plugging in HDMI

I could use some help. I have a frustrating situation where my new Lenovo Y480 crashes (BSOD) whenever I attempt to plug in HDMI which is connected to Yamaha RX-v567 receiver.
- If I bypass the receiver and hook directly to the TV, no problem.
- I can plug right into my upstairs TV with HDMI also, no problem.
- I've updated both my NVIDIA and Intel video drivers.
- I've updated my windows system updates.
- I went here and tried to use the viewer and enabled mini-dump collection but can't make heads or tails of it: Blue screen of death (STOP error) information in dump files.

I'd include the dump but don't have any idea where it's at...

P.S. My old Lenovo Y470 and my wife's Gateway laptops work(ed) fine through the receiver. My OLD Sony Vaio didn't work through the receiver but didn't result in BSOD.

I'm stopped at step 3: generating System Health Report. After the Resource and Performance Monitor runs, I'm unable to save report b/c I get an error message: "An error occured while attempting to generate the report."

I'll have to google why this may be happening later; I have to go to work right now. I wonder if it had anything to do with changing Control Panel -> System and Security -> System -> Advanced System Settings -> (Under Startup and Recovery) -> Settings -> (Under While debugging information) selected "small memory dump?" I made this change to try and use the BSOD viewer as discussed in first thread.

I searched Google for last 1-1.5hrs and cannot find a solution to not being able to run perfmon /report. Any other ways to approach this?

I've completed SFC /SCANNOW. No issues there...

OK. I updated my specs at the link and am attaching the jcgriff2 folder.

I would add that as I was attempting to collect necessary information for my specs, I noticed my "Logitech Driver Interface" has a small question mark on the icon itself (pic attached). I'm guessing that's not an issue but thought it interesting...

Finally, I came across this link when searching for the code on one of my BSOD read from BlueScreenView.exe: http://support.microsoft.com/kb/2552033 Should I install the hotfix? I should add that for some reason BlueScreenView.exe only shows one dump ~1.5 wks ago even though I keep getting BSOD whenever I try to plug in the HDMI connected to the receiver to see if what I've done has fixed the problem... .Exe doesn't appear to be showing those dumps.

It's not the end of the world if you are unable to upload the Performance Report.

Are you getting a Blue Screen? Or BSOD? A BSOD, would be a blue screen with white lettering all over the screen. Please duplicate the issue and use your camera to take a picture and upload when the problem occurs.

Copy that. I thought it was a separate problem and may draw a separate crowd of experts?

The BSDO, as I'm defining it, is a blue screen with white lettering that only shows for maybe a second after plugging HDMI into receiver before the computer restarts. I can try to take a picture but it'll be awfully difficult with the short amount of time on the sreen...
