BSOD - 0x00000f4 - SSD Undetected Upon Reboot
No specific amount of time goes by before I encounter an issue where my system begins to run very slow, stutters, and then eventually BSODs, forcing a reboot. Upon reboot, I receive a "No boot disk" error. When I restart and look in the BIOS, my SSD (boot drive) is undetected. Shutting down and unplugging the power cord from the wall fixes the issue but obviously only temporarily.
System:
Windows 7 Home Premium 64-bit
NZXT Phantom Case
ASUS P8P67 WS Revolution 3.0
Intel i7 2600k
8GB Mushkin DDR3 2000MHz
2x EVGA GTX 570s
Western Digital Caviar Black 1TB @ 7200RPM
Mushkin Callisto Deluxe 40GB SSD
Rosewill Lightning Series 1000W PSU
ASUS DVDRW ODD
Zalman CPU Cooler
Logitech G930 Wireless Gaming Headset (USB)
Logitech G700 Wireless Gaming Mouse (USB)
Logitech G15 Wired Gaming Keyboard (USB)
Logitech Pro 9000 Webcam (USB)
Foreword:
System was running fine for 1 1/2 years before any incident. PSU died completely. Sent PSU in via RMA and received replacement. Installed the replacement, system ran fine for ~ three days. Issue began.
Issue (See first paragraph)
Steps Already Taken:
* I have sent in all of the following pieces of warranty / RMA replacement:
-PSU (Only after the first one died)
-SSD
-MOBO
-CPU
-RAM
* SSD is updated to latest firmware version.
* BIOS flashed to latest version.
* I have done a fresh install of Windows (multiple times) after running Killdisk on both drives.
* Changed the outlet the computer was plugged into, including adding a new surge protector (now separate from monitors).
* Tried running the SSD in IDE mode, same issue.
* Hot-plugging is enabled, same issue.
Additional Notes:
Upon reboot and entering BIOS, mouse does not work unless I unplug and then plug it back in.
Mouse wireless connection is plugged into USB port on the front panel of the case while the hard wire is plugged into the rear, directly into the MOBO (same applies to the headset).

Any and all help will be grateful, please and thank you. I have been dealing with this for three months now (with all the RMAs and fresh installs) and really need to have my rig up and running properly as I'm starting to really get behind with things.
