Do you have an SSD? If yes, have you updated to latest firmware?
Your ATI card drivers need to be updated. Your current ones are of 2010.

Make a hard drive test from the hard drive manufacturers website
Run Disk Check on your hard disk for file system errors and bad sectors on it:
