Just a few questions. Do you have the latest firmware for the SSD and the latest motherboard BIOS? In BIOS is your sata controller set to AHCI and is the drive set to hot swapable? Do you have an empty mechanical hard drive? Also, what CPU cooler do you have and have you ben monitoring CPU temps?

Update. OCZ did release another firmware update from 2.22 to 2.25. I installed and things seem to be stable right now. BIOS up to date and set to AHCI. Drive is not set to hot swappable. Should it be? CPU cooler is Hyper 212 EVO. Temperatures have not been a problem. I set power to no sleep. I stressed with Prime 95 overnight without issues. I will use as normal and see if I have more issues.

Hot swappable is one of the advantages of AHCI, but it is not required. I have it set that way, but I do things with my computer most people don't do. If you begin having problems again, remove the SSD and install Windows to the mechanical hard drive. You don't have to do a complete set up, just enough to make it usable for a while to determine if the SSD is the problem or not.

After 5 days of normal operation, I have had no further BSOD's or other problems. It looks like the OCZ Agility 3 firmware update to 2.25 finally fixed this issue after 4 months of pulling my hair out. Thanks so much for the assistance you both provided. I learned a lot. I am marking this problem solved.
