BSOD during random actions, APC_INDEX_MISMATCH most common error
Hello!
I am new to the forums but not to computers, been lurking these forums for a while for general info and good reads...
I have seen 4 BSODs since I did a fresh install of Winodws 7 Home Premium 64bit because I got a new HD through warranty exchange the other day. Also in the last 2 days I have noticed that if the computer is sleeping and I go to wake it, the monitor comes back as no signal. Computer running fine; fans, CPU, HD, etc cranking away just no signal. Only way to fix is on a reboot....I do believe these are related but I am not sure, it very well could be a power issue and I might just have to get another power supply... but that is the least of my problems at the moment.
System Specs:
Windows 7 Home Premium 64bit
AMD Phenom II X4 965 3.4 GHz
6GB RAM
ATI Radeon HD 5700 Series

-Last two BSOD have been directly related to this file, ntoskrnl.exe and the first two had to do with the following three files... atikmpag.sys, dkgkrnl.sys and dxgmms1.sys.
-Error message is almost always APC_INDEX_MISMATCH or KMODE_EXCEPTION_NOT_HANDLED
-Notice whenever I play League of Legends, almost always crashes once during that, other times it is random
-Since yesterday everytime computer sleeps, it does not "wake up" fully, no signal displays on monitor
-Error codes... 0x00000001 for APC_INDEX_MISMATCH, 0x0000001e for KMODE_EXCEPTION_NOT_HANDLED and 0x00000116 for the first two BSODs

EDIT 2: ADDED A WORD DOCUMENT WITH A SCREEN SHOT FROM BLUESCREENVIEW WITH THE LAST BSOD INFO.
