Your logs show your Nvidia Graphics Driver is failing:

Usual causes: Device driver, ?Memory
Download and install this stable driver: Drivers | GeForceCode:Faulting Driver: nvlddmkm.sys Refers to: Nvidia Graphics driver Dated: Thu Aug 30 16:50:16 2012
Then disable the Nvidia updater:
1. Press "Win key + R" combination to open the Run search box.
2. Type msconfig and click OK.
3. The System Configuration window will open
4. Select Start up Tab and uncheck any Nvidia update programs present, click Apply.
5. Select the Services Tab, and check the "Hide all Microsoft Services"
6. Uncheck any Nvidia Corporation boxes, click Apply then OK, reboot.
This will stop Nvidia from updating the driver to the lateset version, which is buggy.
