APC_INDEX_MISMATCH, Typically crashes after random USB disconnect

Attached is my BSOD for APC_INDEX_MISMATCH, it typically occurs after random usb disconnects/reconnects. Any help is appreciated!

Update your Focusrite audio USB 2.0 driver:
fffff801`f6f34000 fffff801`f6f56000 ffusb2audio Wed Sep 25 15:40:36 2013 (5242E7D4)

If you still need more help then you have to follow the Blue Screen of Death (BSOD) Posting Instructions.

Focusrite claims that I have the latest driver installed. I tried uploading the BSOD using the above instructions but Avast said that the download was a malicious file, I assume I can trust the link? I just want to make sure before regretting it.

Also, while checking for out of date drivers, Uniblue claims that I have 13 outdated drivers (Intel P67 Express Chipset Family LPC Interface Controller, PCI Express Port (6,7,8), Storage Controller, etc.) While another driver scanner, "Slimdrivers" says I don't have any out of date..

Here are is my mobo driver list ASRock > P67 Extreme4 Gen3

I am running Windows 8, and all of these look out of date to me :/

Try removing the soundcard and uninstalling the driver to test if you still get BSoDs.

I completely uninstalled my audio driver and reinstalled the same version, to see if the driver I had was corrupted or not. I did not realize that there was an eightforum, sorry! Any idea what is going on with the two different driver check applications? I will try that forum if the issue persists. Thanks Tobias

Those driver update programs have their own separate databases of varying quality, if there are no entries for new drivers for your hardware in the database they will report that all the drivers are up to date. I've had some bad experiences with similar programs so I don't use them myself.
