BSOD error, Graphic Rendering ,NVIDIA graphics,Kernel, STOP0x000000116

Hello, I ve been experiencing this problem around 2-3 months

Whatever game I play after few seconds some graphics start to change and corrupt. If im lucky it turns completely black and game closes itself and returns to desktop. And gives this error :

If I am not lucky, it gives bluescreen error and restarts the pc :

    After restart windows reports :

    Problem signature:
    Problem Event Name: BlueScreen
    OS Version: 6.1.7601.2.1.0.768.2
    Locale ID: 2057

    Additional information about the problem:
    BCCode: 116
    BCP1: FFFFFA800BB85010
    BCP2: FFFFF8800FB8BD1C
    BCP3: FFFFFFFFC000009A
    BCP4: 0000000000000004
    OS Version: 6_1_7601
    Service Pack: 1_0
    Product: 768_1

    Things I tried so far:

    * Format - didnt work
    * Driversweep cleaning and re-installing drivers - didnt work
    * Used some old nvidia drivers - didnt work
    * After format no windows update or driver update, - worked but I cant get any performance from it with Nvidia 267.54, this is not a solution. I cant play most of the games that I could play before. Very low performance even in lowest graphics.
    * Underclocking - didnt work, I did it to everypossible number, rendering still happens however can play longer it is randomly and takes more time to crash. Mostly around 15-30min (without underclock max 30 seconds) still not a solution. Also tested in heaven benchmark, when underclocked there is still rendering but no crash or bluescreen, with normal settings max 20 seconds and blackscreen or kerner error.
Windows kernel fix, didnt work, it says I already have that file.
    Disabling-enabling optimus(from bios) - didnt work
Disabling Nvidia 540m - didnt work, no crashes but no video card also I can play non-graphical games with Intel 3000 without any crash.

It does not crash while playing videos etc. But no chance in any game.
There is no heating problem. I can play for hours without updating any driver, however with 267.54 it is not possible to play most games. I am testing it with skyrim and deux ex. No chance. 5-9 FPS
Directx, framework etc all lastest version. Nvidia Physix also.

    I have Lenovo IdeaPad z570;

    intel i5-2430m 2.40ghz
    6gb ram
    Nvidia 540m 1gb
    Windows 7 home basic sp1 64bit

Tried every possible thing, I just dont understand how I can play with basic driver but not the updated one. Nothing works. Also most of the time I hear some sort electrical voice and the sound of the game or music stuck and replays it while rendering but it shuts up when game is closed(or crashed)

    help

    okay,

    Code:
*** WARNING: Unable to verify timestamp for nvlddmkm.sys
*** ERROR: Module load completed but symbols could not be loaded for nvlddmkm.sys
    Probably caused by : nvlddmkm.sys ( nvlddmkm+98bd1c )
    They are you nvidia drivers.

    The error code was a: VIDEO_TDR_FAILURE (116)

The first thing you need to know is that this error pertains to an issue with the video card, and that your display driver failed. The display driver in your video card could be the problem, indicating you need to update the driver.

    Drivers - Download NVIDIA Drivers

Overheating of the card and dust can also be another source of the problem. Making sure the card in the case is free of dust with canned dust or is properly cooled with a fan can help eliminate VIDEO_TDR_FAILURE errors.

Another cause can be the failure of the hardware itself, meaning you may need to purchase a new video card or possibly other components relating to it.

Please Run a Stress Test on your GFX Card.

Video Card - Stress Test with Furmark

Watch for artifacts on the screen, and keep an eye on the temperature log. If you see artifacts, or the temperature levels out, stop the test.

Also please can you test your ram.

    RAM - Test with Memtest86+

thanks for your post, my stress test fails, my record is 1 min, graphics starts to corrupt and max 10 seconds after that it gives this error:

and after that furmark : not responding.

I tried almost every driver version 1 by 1, there is no difference. I guess it is hardware problem? but I cant understand how it works after format without updating driver.

    * In addition to that, underclocking makes the tests go more than 15 mins maybe more, does that mean there is something do with hardware? because the less clock speed , longer test/game.

Okay make sure the GFX card is clear of dust, and is seated in the slot properly.
