I too get the bottom screen, but I can assure it is because mine can't handle it.
Mine is a work PC from 2,5j, no way it can render perfect.
My point simply, even though it renders not fine, it does not mean the GPU is bad. This is the reason I sometimes ask about data outside of the stress tests.

Interesting......well I plan on upgrading the graphics card with a good black Friday / cyber Monday deal. I think (hope) that will help the flickering/freezing issues.

The drivers you mentioned above are up to date.

Could you please answer my 2 questions?

Havee you done something with DriverMax setup?
How is it with only 1 monitor?
According to the event logs, there are several error codes.
A quote from usasma about several error codes

The differing error codes are usually symptomatic of a lower level problem within the system. They are usually caused by one of these things (the list is not in any sort of order):
- borked (broken) hardware (several different procedures used to isolate the problem device)
- BIOS issues (check for updates at the motherboard manufacturer's website)
- overclocking/overheating - You'll know if you're overclocking or not. If uncertain we can suggest things to check.
- dirt/dust/hair/fur/crud inside the case. Blow out the case/vents with canned air (DO NOT use an air compressor or vacuum as they can cause damage to the system)
- missing Windows Updates
- compatibility issues (3rd party hardware/drivers), older systems, or even pirated systems
- low-level driver problems
- or even malware (scanned for when we ask for hardware diagnostics from Initial Hardware Diagnostics or Hardware Diagnostics ).

Been using single monitor all morning. Program flickering doesn't seem to be an issue.

DriverMax isn't installed. I stopped using it after you said it could cause problems.

Newly seen BSOD: APX_INDEX_MISMATCH

New logs attached. Also, most of then new BSOD's happened when watching streaming videos.

Correct me if I'm wrong, but according to my search results your PSU is not good enough.
The CPU takes: 95W,
The GPU takes: 400W,
The Seagate Barracuda 3TB 3.5" 7200RPM takes: 8W.
This all together, just a few components, is more then 500W.
