Next time just add it with the other files, that spreads the files less making analysis much easier.

Please uninstall everything of Nvidia using Display Driver Uninstaller and install new drivers from Dell. Be sure the clean install box is checked and only install the Graphics driver and the PhysX driver, you can use this tutorial to do so NVIDIA Drivers - Avoid Problems

Alright, IMO it would be best to replace the hard drive as it looks to be failing.

Is your laptop under warranty? Based on below I would assume it is, unless it is a clean reinstallation.
Code:
Original Install Date: 20-03-2016, 11:22:19
System Boot Time: 21-08-2016, 00:35:10

No its not under warranty. I bought this in 2011. Is there anything we can do apart from replacing hard disk and the BSOD is due to failure of hard disk?

For some things a clean install of Windows with afterwards a proper chkdsk will help when there are problems with the hard drive, but in your case the physical problem is included. A hard drive that goes south with reading/writing cannot be fixed with a clean reinstallation.

Most likely yes.
