Icon error using Devices and Printers Metadata Authoring Wizard

I'm trying to create an icon to use with the Devices and Printers Metadata Authoring Wizard. I'm using IcoFX to create the icon(s) and have no problem creating the icon with all 13 size/color depths needed, but when I drop it on the box in the wizard I get the error shown in the attached image.

Anyone run into this before? I've tried using the original image saved as PNG, BMP and JPG, but the output is always the same. The error message doesn't show the 256x256 32-bits + Alpha, so it may mean those color depths are carrying forward the transparency but the lower color depths aren't?

Any ideas would help. I've Googled it to death and really haven't found an answer. I've tried the process from the thread How to Make a Icon with a Image Published by Bongo back on 06 Jan 2012, but it didn't do any better. I've got GIMP and Photoshop available, but not sure how to get these icons' transparency set.
