It's called rocket dock and it's free. There's a whole thread about it under customization.
You'll find it through google. It comes pretty plain but you can drag and drop your icons into it.

It's saved so the computer knows what to use.

Yes I know but that just stores the image, some images posted above have to logon details included. Or are they just very good photos of the screen?

Some screens are manual captures with cameras, some are the actual images and some are screen captures taken from a VM (virtual machine)

Here you go, and, no, I didn't make it, downloaded it from somewhere, can't remember where though.
