downloaded a win7 theme and cant get desktop icons

Recurring loss of desktop icons Win7. I am able to restore my desktop icons by removing IconCache.db then killing explorer.exe process. On rebooting my icons have reappeared....

I see checkboxes in a green square box at the left bottom corner of desktop icons (for xls, txt, jpg etc.) in Win 7. This is annoying. How do I remove this? Is it a malware? I tried Start > Folder options > View..doesn't work. Thanks in advance for the suggestions.

I recently installed Win 7 Ultimate on a new build. Whenever I reboot, the desktop icons keep rearranging themselves alphabetically along the left side of screen instead staying where I put them.
