This sounds to me like the graphics card physically is going bad on you & not driver
related at all.
Try running a 3DMark Vantage
benchmark on it & see what happens.
If it happens during the bench testing or freezes or crashes this is more than likely the problem.
Also it could be overheating...
You can check the temps by installing EVGA Precision
which is basically Rivatuner redone for EVGA.
Great for OCing you card as well.
