That is a known issue with some Realtek drivers, I have had it happen a few times and it was always the Audio driver.

I had the same problem... installed CoD4 via Steam in Windows 7 64-bit... single player worked but multi didn't. So I plugged in a microphone and BAM! It works for me, too! WTF???

    FOR A MORE PERMAMENT SOLUTION:
Try right clicking on the volume control icon, and click on recording devices.
Right click somwhere in an empty space in the white box and click show disabled devices. Right click on stereo mix and click enable. It worked for me, now cod dosnt need mic to run! (I had previously installed realtek hd drivers though, i dont know if that makes a difference or not)

    Try this for COD4 Mulltiplayer

    MY SOLUTION WORKED FOR ME

I have the Asus P6T SE with HD Audio and Realtek soundcard. COD4 single player works fine, multiplayer started and closed to the desktop.
The problem was solved in two ways.
    1. If you have front panel audio put a mic in the front panel - note mic only.

2.More permanent fix - disable front panel jack detection in the HD Audio Manager - see pic below

    Worked for me!

    Problem Solved

I had the exact same problem, It's a glitch with the game on Vista and 7, If you plug in speakers and a Micraphone the multiplayer game loads fine.

    How random

My single player campaign is running fine but i have a little problem with my multiplayer campgain. When i start the game ( multiplayer ) it shows the splash screen and then it goes in to the main menu. About two seconds later a error message apears with the following " couldn't load image '~weapon_ak47_s-r-61g-61b-61&w~0848fdf9' " Thanx in advance,

This works for me fine, didnt have to install any extra drivers or anything and now multi loads fine with no crashing back to windows. This was driving me up the wall so cheers mate you've made my day!! Couldnt take multiplayer MW2 for much longer.

AVG doesn't work with Steam, and if you have that it could be the problem. It's a known issue with VAC I believe?
