Windows 7: GTA4 EFLC.exe crashes during gameplay

Am currently loving playing GTA4: Episodes from Liberty City. The game runs and loads well (offline/single player), but crashes frequently during gameplay at seemingly random times. Included with my siryandi.zip system information file is a report of one of the crashes.

A secondary issue, the game crashes within 10minutes every time when playing online multi-player mode. This could be due to my poor internet... I'm more concerned about fixing the single player anyway.

I've had no luck on steam forums or elsewhere, and just can't seem to fix it myself.

Got the legit copy of GTAIV + EFLC from steam. All game files integrity verified to be correct.

Not an overheating issue, case is cool. I prefer to opt for FPS over maxxing out

All drivers updated to most recent versions manually checked using "Driver Reviewer"

I've had issues in the past with other games, plus occasionally my display driver stops, glitches and sometimes recovers. Something that I can't seem to localize has been wrong with this PC for over a year, and so here I am.. please help!
