Well I tried the d3d9.dll fix, and it works for me. The lag is pretty much gone and the game plays as Fallout 3 did.

Touch wood, FNV has played exactly like FO3 right out of the box.
The only issue in 9hrs I've seen so far is NPC's getting stuck in rocks etc.
Other than the issue I noted about the save/continue system, I agree. Though I've got less than 2 hours in the game thus far.
As a side note.... The game basically looks and feels the same as Fallout 3. In fact you could argue that this is a large expansion of Fallout 3, which is unfortunately contributing to my low play time
I agree. but once you get to the area of Primm things start getting better, at least IMO.
My Quick/Auto save started bugging out again
Its only a temp. fix, even with all CLOUD options disabled.
But from what Ive read on the official forums, its not actually eating them, just naming them wrong.
In Documernts/My games/ FONV you see you quicksave and auto saves.
2 of each, one with a .bak extention.
If you delete the auto.fos and quick.fos (These are the old ones) then remove the .bak extention from the other, renaming it quick.fos or auto.fos, youll have your real save back.
Ive tried everything and thats the only thing that actually wortks for me. but it seems the devs are aware and working on it now.
Ive had a few small issues. Mostly as mentioned, creatures falling into the world etc. on occasion.
Also, When 3 or more NPC are around shooting at me, framerates seem to plummet. Many seem to have this problem as well, although Im not sure if its the game itself, or a ATI issue.
Forcing AA and AF through CCC seems to help.
Tbh, I expected it to be what it is - more of the same, so I'm not surprised/disappointed on that score. Although with the similarity it is a bit of a grind to start from scratch with the levelling up.
I was expecting more from the hardcore mode though. Atm it's more of an irritation than an immersion.
I've noticed the slowdowns as well, but haven't had Fraps running so I can't say exactly how much. It does recover quickly however. I'm running with CAT A.I disabled to switch off the 2nd GPU, so that may or may not make a difference.
It hasn't been enough of an issue to visit fiddle/tweaktown. Yet

I'm playing Fallout New Vegas, no issues at all, I haven't encountered any bugs so far. I'm actually enjoying it. I'm doing side quests and it's pretty fun. Also, the map is pretty huge. Lots of stuff to see.
There WAS a thread over at Bethesda Softworks Forums -> Fallout: New Vegas PC Issues about issue with SLI/Crossfire, but it's either been deleted or moved somewhere else as I've looked high and low and can't seem to find it now.
