Freezing on the shutdown screen.

When I go to restart or shutdown my computer freezes on the shutdown screen, It does it in safe mode also. I reformatted my hd and did a fresh install of windows and it is still doing it. Also after awhile of sitting idle my monitors will go black but my computer is still on, and I have to use the button to restart it.

When I run speed fan S.M.A.R.T test, I get a read failure and this.

When you install Windows, the absolute first thing you do is go and download the motherboard drivers, than all the other drivers:
  • GPU
  • Peripherals
  • SSD
  • etc.
  • (Not in order)

Also have you tried to find the latest BIOS update for your motherboard?

Could be a bug that may have recently got patched as it seems weird that you still have a problem when you have done a fresh install.

I believe you're on the right track since the HD SMART cannot be ignored.

Next I'd run the HD maker's HD Diagnostic extended CD scan, followed by a full Disk Check.

Back up your data as the HD may be failing.

Drivers are handled differently in WIn7 than XP. I would not change out any given by the installer or optional WIndows Updates unless performance points to one. I would also not update the BIOS unless the latest version has improvements you specfically need.
