Is the white piece of paper with the red "x" across it standard?

Yes that is a standard Delete symbol/ Red X.
Now the white paper could be a couple of things,
One the shortcut is broken and a thumbnail is not available,
Or there is no Default program associated to open the file type.
The cache rebuild link would have cleared up the issue most likely.

Hello jicq, and welcome to Seven Forums.

In addition, you will only see the Recycle Bin icon in that dialog box when you are moving/deleting a file to the Recycle Bin instead of permanently deleting the file bypassing the Recycle Bin.

Hope this helps,
Shawn
