EXE file icon missing

So I had that nice blank page style icon before, on files that doesn't have a specific program + it's specific icon assigned. But now I have this http://www.freeimagehosting.net/t/fwj9x.jpg How do I fix this?!?

Can anyone help me with this problem..the yellow icon folder is ok when i open a different folder names and files but when im browsing the net to upload pictures my icon folder is missing the yellow design logo.. http://img9.warez-bb.org//images/smiles/icon_sad.gif and if im using photoshop...
