I figured out the issue. I had an hdmi cable plugged into the graphics card and had it running to my plasma TV. For some reason when I updated the graphics card drivers, it then detected the hdmi connection and made the TV the default display.

I was able to change it over to using my monitor, and everything looks great.

Lesson learned - if anyone else encounters this - don't plug in an hdmi cable until you've finished reinstalling and updating Windows.

Thanks for reminding me about my other monitors! After fixing an issue with my corrupted hard drives, my main monitor was dual connected to a laptop. I was expecting Windows to appear on the monitor as my safe mode monitor when I was booting up.
