Windows Password Turns On By Itself

I have Windows 7 with 2 user accounts, neither account was set up with a password. When user Ken goes to log into his account Windows asks for a password which was not set up. We than log into the other user and go into the control panel. From there we go to User accounts and turn off the password for Ken's account. He than can log into his side for several days than he goes to log in and again it asks for the password. We have turned off the password to his account maybe 5 times in the last couple of months but it keeps coming back on. I'm wondering if anyone else has had this issue?

Work through the Troubleshooting Steps for Windows 7 starting with check for infection, establishing a Clean Boot, and scanning the logs specified for repeat errors or clues near the time of the anomaly.

Compare the install you did with the perfect install compiled in Clean Reinstall - Factory OEM Windows 7 which steps are the same for retail.
