Display Drivers Crashing

Hello.

My issue is the following:
-while playing games the display driver just crashes after 5 minutes of game-play, after which I have to reboot in order to be able to play another 5 minutes of the game before it happens again
-while browsing the web, it crashes and recovers
-while watching videos the driver crashes

My setup:
Processor: Intel(R) Pentium(R) Dual CPU E2180 @ 2.00GHz (2 CPUs)
Memory: 2046MB RAM
Video Card: NVIDIA GeForce 9800 GT
OS: Windows 7 Ultimate 64-bit
Motherboard: GA-P35-DS3L

I have tried memtestx86 and I've let it run 27 times and it gave no errors.
I have tried sweeping all of the drivers and the leftovers and installing new ones.
My drivers are up to date and downloaded from the nVidia website.
I have checked for malware.
I have reinstalled windows.
I installed the newest motherboard drivers.

None of what I did above helped and this is really getting annoying now as I can not do anything on my PC.

If you have any suggestion, please help, thank you.

EDIT: The graphic card temperatures are in a normal range.
What I have also heard is that it could be a failing PSU.
If it's of any help, all components except the graphic card are 5 years old.

please check it, because it could probably be the problem.

Check also how many Amp you have under 12V specs.

Your PSU should be OK...
Are you overclocking the card?

Hello, thanks for the response.
I've tried the GPU:OCCT test and these are the results.
The GPU:Memtest is grayed out so I can't perform it.

GPU:Memtest should work on the 9800GT unless you haven't installed the required driver. Your standard GPU tests show no bad results.

Try to use & replicate the error with another card, you'll know for sure if the fault lies in your 9800GT.
