Depends on the game. Some can be changed in the INI (if available), and some by using command line edits; like Battlefield 2 before they added WS support in the last patch 9says demo, but same applied to the full version),

Battlefield 2 - WSGFWiki

BF2 gets it's WS support through a Command Line edit:
"C:\Games\Battlefield 2 Demo\BF2.exe" +menu 1 +fullscreen 1 +szx 1920 +szy 1200
If you can set it via the INI method, make sure to have the rest of your in-games settings how you want them in game, then make the resolution change in the INI and set the file to "read only" so it can't be reset when the game is run.

As for why it's happening, this,

Among one of the games I play is LOTRO, so no console there. Is there a free program that controls the resolution of games out there?
is probably why more than anything.
I agree, if we can find the answer to this specific question then the problem is most likely going to be solved.

Can't increase resolution/refresh rate
My monitor is 16:9,the GPU control panel showing my monitor capable to go up to 1920x1080 75Hz,however i cannot go beyond 1600x900 60Hz,also i can 1600x900 75Hz,but everything becomes blurry. Why i can't go beyond 1600x900? Why 75Hz is blurry while 60Hz looks fine?
