Get these fans and put it under your graphic card, it's also very very easy to make your own, I made myself 1 yesterday, haven't tested it yet cause I didn't get my PC back from maintenance yet..

Screwed? No, but you are approaching what that card can tolerate... and are running outside of my comfort zone.
Personally I'd buy an aftermarket heatsink and fan and do anything to increase case flow. How much room you have to play with?
You can definitely work with that... no dremel needed. Get some air pushing past that bad boy.
Factory cases are poor - I picked up a cheap Antec 200 that has a 140mm exhaust at the top and 120mm at the back - it also has 3 mounts for another 3 120mm TriCool fans. Going to mount one on the side. That temp isn't normal. My 4890 never hits past 70 degrees celsius - although the PSU is mounted on the bottom blowing air up past a PCI card.
What does it have now in the way of fans?
A picture or two of inside the case would help (couldn't find any when searching). It may just be a matter of adding another fan or two (or better ones than what are there now, and possibly a bit of cable management (OEM's are notorious for leaving it a right bird's nest) to help improve airflow.
