Getting abnormally high access times, in excess of 30,000 ms, according to Resource Monitor. Should I be worried? Tried to do a defrag but didn't help. There's often a soft fast repeated tapping noise coming from my HDD.

That sound is usually indicative of a crashing hard drive or soon to crash. Back up all you valuable data and get a new drive.
i agree. WEIRD sound, noises, ticks from any component that contain moving parts is an indication that the component is failing. backup before its too late
