Have you tried uninstalling the video drivers and trying different drivers, sounds like that is at least part if not all of your problems.

Yes, I have tried the latest drivers from NVIDIA as well as the drivers offered on the ECS website. I don't know which, if any, version of the drivers would be better than either of those.
Try reinstalling your video card. Try another video card.
Check all cables and connections.
Uncheck everything in msconfig>Startup except AV and gadgets/stickies.
