Screenshot reveals a file is incorrect in chkdsk/ repair?

I ran chkdsk in safe mode and the screenshot reveals what came up .

Problem is when i ran chkdsk/f/r in regular mode it said no problems found , any ideas?

Did you reboot so chkdsk could run after you entered chkdsk /f/r ?
