I'm Getting This Screen After That and Restart or Freeze!

Hi, in these days im getting this screen while playing games or surfing on internet. After that pc freezing or restarting and also sometimes its getting bsod. i used memtest and tested like 1hour 30min no error. changed ram slots and get this screen again nothing changed at all. i got ati ccc 12.6. formatted my pc 3-4 days ago i mean its clean like baby. installed all windows 7 updates.

What could be the problem?

You don't have all the necessary files attached. Please run the BSOD Dump & System File Collection app from the below link and upload the .rar file.

Have you tried using a different monitor?
