Volume Mixer Audio Output Indicator Gone

You know when you are playing an audio file and you are able to see the current output volume of the song in the Windows7 Volume Mixer as a semi transparent bar behind the volume slider?

For some reason, mine is gone.

Also when I put headphones in my PC it is really loud and I have to keep my volume levels really low.

Please help me with my problem.
