I see four instances of the AMD High Definition Audio device and probably the problem.
You failed to fill ouit the MY System Specs, but I assume you only have one video card.
Suggest you go to the Device Manager and Uninstall ALL the entries for the Video Card (if there is multiple entries). RIGHT click on an entry and then LEFT click "Uninstall". DO NOT check the box to also uninstall the drivers. Do this for each Video card entry listed. You will probably lose the video resolution with uninstalling the video but that will recover. After you have all of the video entries uninstalled, restart the PC and when Windows starts it will detect and reinstall the Video (you may have to reset the resolution after its reinstalled). Then check the playback devices and see if you only have one HDMI output.
