Solved volume problem
Right click on speaker icon on task bar
choose "Playback devices"
select your speakers and choose "properties"
choose the "Enhancements" tab
Check the "Disable all sound effects" box
Job done :)

thats a negative. I started with all enhancements disabled and mine did/does it. As someone mentioned it seems to have something to do w/ onboard sound and not sure if anyone is using anything other than Realtek w/ this problem - but it may be a realtek issue specifically.
have tried all fixes here and nothing. at first i thought it was a bad mp3 but tried a youtube video and pfft. was testing out my new 7.1 headphones
Also having the same trouble but it started about a month ago on both my computer and my wife's computer which are almost both the same builds. The only thing that changed was we got new video cards recently. Just incase this may have something to do with it, what video cards do others that have this problem have? I have a Gforce 660.
Ah yes, I had this problem too. I found a solution though! Press , type sound and select "Change system sounds". Then you go to playback, the device you want this to work on, right click and go to properties. Go to advanced, and deselect "Give exclusive mode applications priority". Click apply, and voila. Some programs may run into some problems so it is safe to restart.
