What's the primary difference between the free versions of WinPatrol and the Plus version that costs $29?

It is important to note that the $29 is a one-time license fee, with no renewals. You can also use that license on all the computers in your home.
The following additional features are included in WinPatrol PLUS.
I recall the performance increase with PLUS as being significant. The PLUS Knowledge base is very useful. Controlling ActiveX and the custom registry monitoring features are also very helpful. It's the Real-time Infiltration Detection which is what alerts you immediately when something tries to install or makes changes on the system I take it.

Many WinPatrol features have the option to set the time between Scotty's patrols. Scotty will patrol in "real time" for WinPatrol Plus subscribers. This means that if there is a change to a monitored feature, immediate notification will be provided. With the free version of WinPatrol, it is up to the user to set the time between patrols.  Anytime the clock icon is available, the time can be adjusted between patrols. Clicking on the icon opens the option windows illustrated below.

You can have all the anti spyware/av's you want on your system. But one thing prevails above all...common sense.
If you go to questionable site, or go looking for pirated software or other things, then the sites you'll encounter will more then likely be laced with viruses or malware.
That's not to say that if you stick to "legitimate" sites, you'll be 100% safe, legitimate sites have been infiltrated.
Just use a bit of caution, be suspicious of "erratic behavior" if your PC starts acting in ways it didn't before, and always keep your AV/Spyware defs up to date and use them. They do no good if they're sitting on your HD not being run.
For my part, I use Norton 360, Comodo firewall, MSE and run scans of Malwarebytes on a regular basis. I also randomly check my processes using Process Explorer and use TCPview to keep an eye on the connects.
And even though Norton and MSE have background scanning, I still do a full scan with both at regular intervals.
Edit: Secunia is a good program for helping you keep tabs on what needs updated.
