A file on my portable hard drives says it requires system permission but when i give it system permission it still doesnt let me delete it, it bother me, ive scanned it with 3 virus programs and it's not a virus.
Have you tried right-clicking that file and choosing Take Ownership then afterwards, try to delete it? That's what I would do first.
In my context menu, it does exist. You can try however, using programs that forcefully deletes files. Check these out:

files can'tt be shred because they are in use
It says the file is a Windows Installer Patch so I think, you don't need to delete it if its used by the system but if its a system file, what the heck is it doing on your portable hard drive?
OK, now the next step I would try is open Task Manager.
Right click on the Taskbar then Start Task Manager. On the "Processes" and "Applications" tab look for the NDP40-KB2518870 thing in there, right click it and choose "End Process" then afterwards, delete that thing in your portable hard drive. You can also try renaming it first before deleting.
However if its not causing any issues, just leave it there, better not delete it.
