Hello OptimusOne

Try this - Icon Cache - Rebuild

Depending on which icons you have replaced you may also need to change them in the imageres.dll too

A lot of icons are in imageres.dll as Danny said but not all of them, if you post a few screenshots of the ones you want changed we can most likely help you.

Looking at that theme I can see the creator did not provide a imageres.dll with those icons in it that is where you will find all of them. Do you have the icons you want to use there?

Icons for certain programs are getting replaced with others in taskbar
Dunno why this is happening but for some reason the programs Quicktime and Adobe Reader are taking on the icon of some other random program in my task bar. I tried uninstalling and reinstalling both but it didn't seem to work. Is there anything I can do to fix this? It's really strange.

Some icons disappear and are replaced by generic ones
For some reason icons for certain programs will disapear and be replaced with the 'generic window' windows icon. I'll upload a screen shot. First it happend with opera. and now with d3doverrider. Additionally I think I was using the program when it happened, so that may be contributing to the cause.
