    NoelDP,

I downloaded this folder to the default location (no location was specified in the directions). I then (as per your instructions) extracted all. This created the folder.

    Code:
    Microsoft Windows [Version 6.1.7601]
Copyright (c) 2009 Microsoft Corporation. All rights reserved.

    C:\Users\John>cd downloads

    C:\Users\John\Downloads>dir pvgaa
Volume in drive C has no label.
     Volume Serial Number is A01A-E001

     Directory of C:\Users\John\Downloads\pvgaa

    01/07/2015 06:26 AM <DIR> .
    01/07/2015 06:26 AM <DIR> ..
    01/03/2015 05:15 AM <DIR> Packages
                   0 File(s) 0 bytes
                   3 Dir(s) 114,867,445,760 bytes free

    C:\Users\John\Downloads>
The newly created folder has a folder in it called packages.

    Code:
    C:\Users\John\Downloads\pvgaa>dir Packages
Volume in drive C has no label.
     Volume Serial Number is A01A-E001

    C:\Users\John\Downloads\pvgaa>
I copied this folder to the Directory of c:\windows\temp\checksur\packages

This was incorrect, as you specified C:\Windows\Temp\CheckSur\Servicing\Packages

After running the tool, all references to KB3013126 in Checksur went away. However, references to KB3004394 remain. Also, the Checksur file size was reduced from a zip'd 24MB to much smaller, 31KB.

I have recopied the packages folder to the correct, specified folder:

    Code:
    C:\Windows\Temp\CheckSUR\servicing>dir packages
Volume in drive C has no label.
     Volume Serial Number is A01A-E001

     Directory of C:\Windows\Temp\CheckSUR\servicing\packages

The Checksur from today and before seem identical, while the CBS shrunk.

    OK - I think I'm sorted now.

    I've uploaded a file - pvgac.zip - to my OneDrive at Noel's OneDrive
    Please download and save it.
Right-click on the downloaded file, and select Extract all…
Extract to the default location - which will create a new folder pvgac in the same place.
Open this folder - there should be one folders inside it (Packages)

Copy the content of the Packages folder to the folder
    C:\Windows\Temp\CheckSur\Servicing\Packages

Now run the CheckSUR tool again (it may take a while)

Post the new CheckSUR.log file, and the CheckSUR.persist.log file.

    Reboot and immediately uninstall KB3004394!!
