Does that need to start with Windows for the fan profile to apply at boot?

Yes, you probably have asus gpu tweak that came with your card's disc. They are all pretty much the same. You can use MSI Afterburner, Precision X, Firestorm or Asus GPU Tweak

    They use little resources

Yea, I use Precision X. But it's close to the same as MSI Afterburner, I think they are all based on Afterburner. All the ones I have used are about the same.

You don't need to start it with Windows, at least I don't. I let the stock fan profile do it's thing if I'm just browsing or on the forums or something, I just made a profile and start Precision X when I am going to game. That's really the only time you should need it anyway.

I'm lazy to do that. Lol. Anyway, at 1215MHz at 72C, it's fine I think. Yea it could be lower, but until I start to add voltage, I think it's fine.
