BUGCODE_USB_DRIVER BSOD at random times

I've been having problems with BSOD for awhile, but it's been getting worse and worse, so I finally decided I needed to do something about it. I've been getting the BUGCODE_USB_DRIVER at random times. I have not figured out anything causing it. I'll just be doing something online and it'll happen. I know there is something wrong with my USB slots for awhile, since they don't always register when I put a flashdrive in. I haven't figured out anything on how to fix that, so I figured these two issues might be connected.

My computer is Toshiba Qosmio.
Processor: Intel(R) Core(TM)i7 CPU Q 740 @1.73GHz
Installed RAM: 6.00GB
System Type: Windows 7 64-bit
I don't know for sure much more than this, but i'll find out anything else if I need to.

Also, I followed the instructions to attached the ZIP folder, but every time I tried to upload it, it said the folder was still too big. I kept getting a "413 Request Entity Too Large". So, I had to break it down into separate folders. That's the only way it was letting me upload it.

Hi Dmack. There are no dump files in the zips you uploaded.

Configure dumps
First, we want to make sure the computer is saving your dump files. Go to Control Panel -> System -> Advanced System Settings -> Startup and Recovery [Settings] Make sure that "Write Debugging Information" is set to Small Memory Dump (256kb) if it is not already.

Next, make sure that the folder C:\Windows\Minidump exists. If not, create it (allow permission if asked). Make sure any dumps you want to analyze, whether yours or others', are in this folder. You can ZIP/RAR the files and send them to someone else to analyze as well.

Security App
Avira can be a cause of BSOD's. Please remove and replace with Microsoft Security Essentials to see if it provides more stability.

Microsoft Security Essentials and Malwarebytes are recommended from a strict BSOD perspective. They do not cause blue screens on the system as other AV products do.

Tip

Make full scans with both separately.

Start up
Keep less stuff at the start-up. Only anti-virus, this helps avoid driver conflicts and improves time to log in to windows.

2. go to power options (you may have to set view to small icons)
3. click change plan settings
4. in the window that opens:

6. click change advanced power settings, expand usb and ensure usb selective suspend is disabled.
