Check to see if you have the latest firmware for your SSD.

Download Center: Download Software, Firmware, Drivers & Manuals | Samsung

Post a screen shot of your SSD with CrystalDiskInfo - Software - Crystal Dew World

Run a Disk Check to scan for errors

       Note

Afterwards go into an Elevated Command Prompt (Run as administrator) and type in sfc /scannow
If it finds errors reboot and run the scan two more times

Run SeaTools to see if your HDD or SSD is failing

    Post back the results.

It passed all the tests without problems...can it be a problem with RAM? or even motherboard? Cuz it had same problems with my old SSD disk, So i bought a new one cuz i thought my old one was failing so it can't be my new SSD failing..must be something else.

I also have latest firmware for my SSD

It could be yes, try this:

    Memtest86 will test your RAM

       Information

Run Memtest86 for at least 8 passes so we get enough information to confirm that your RAM isn't failing
