    Computer keeps BSOD and I have reformatted twice (UPDATED!)


    As title says my Computer keeps BSOD and I have reformatted twice and the problem still persists. I have looked for other quick fixes but none seem to be working so I think I am out of my element and need some assistance form people who know how to assess these mini-dumps. I ran driververifier.exe and it definitely seemed to be overloading everything, but the problem came back when I turned off. I did not get a BSOD when driververifier was loaded (about 36hrs run time). I swapped out my PSU thinking maybe my PSU was shot, but that did not help. I have updated all my drivers, or so I assume, and Still getting errors. They dont happen at any one time either, sometimes its when im in a game, sometimes on chrome, sometimes just waking up from sleep.
    Primary BSOD messages are:
    DRIVER_POWER_STATE_FAILURE
    Just a blank BSOD stating an error occurred.
    APC_INDEX_MISMATCH


    most recent stop error
    0x0000001E(0xffffffffc0000005, 0xfffff80002cb9ac5, 0x0000000000000000, 0xffffffffffffffff)


    Any help would be greatly appreciated, Please let me know if you need any more information.

    The dump blames iusb3xhc.sys (Intel(R) USB 3.0 eXtensible Host Controller Driver), have a look for a newer driver here - https://downloadcenter.intel.com/default.aspx

    The zip only contained a single dmp and none of the other required info so that's the best I can do.

    What more info should do you need? how do I get more dumps in the file

    also keep trying to load that file but no drivers are being located

    Here is all the info sorry the program download before when creating the zip file only included 1 dump that's it. here are 2 forms of most up to date info.

    Its an older disk for sure, not sure how to tell how old but was bought in 2009-ish when i built my first rig
