BSOD after system restore

Heres my issue. Recently I have had issues with my pc freezing up on me. When doing this, I would restart my pc manually by holding the power button down. Well I did this yesterday and it gave me the notification of :Windows could not start because of a computer disk hardware configuration problem"

When seeing this I tried to go into safe mode, starting windows normally, start windows to when it last worked properly. Well each and every option would take me to the windows screen and then a blue screen would pop up for like a second and then it would take me back to the black screen again so that I could make a selection.

I decided to use samsung 5 recovery by pushing f4 and restored everything. I lost all my data, but i didnt care thinking that as long as it worked it was fine.

Well, it didnt work. i installed some programs and when you do, it ask you to restart pc so that proper installation takes place. Well I did and once again it brought me back to the black screen. Once again, I tried to make a selection and once again the blue screen would pop up and take me back to square one.

I partitioned my disks like six months ago because I upgraded my hard drive to an ssd drive so I dont know what the issue is. At this point I cant restart my pc because if I do, I will have to restore everything again in order to get my pc working again.

I followed instructions and here are my zip files. I am hopeful that you guys can help me out.

There are two storage devices. One is a Hitachi Travelstar 5K750 HDD and the other is a Samsung SSD 840 EVO 250G.

Where you have installed windows? On the SSD or on the HDD?

Asking, because there are some discrepancies there.

The HDD is partitioned as .....
Partition Disk #0, Partition #0
Partition Size 100.00 MB (104,857,600 bytes) {SYSTEM RESERVED?}
Partition Disk #0, Partition #1
Partition Size 271.00 GB (290,984,034,304 bytes)
Partition Disk #0, Partition #2
Partition Size 406.26 GB (436,218,101,760 bytes)
Partition Disk #0, Partition #3
Partition Size 21.28 GB (22,846,373,888 bytes)
And, the SSD is partitioned as
Partition Disk #1, Partition #0
Partition Size 400.00 MB (419,430,400 bytes)
Partition Disk #1, Partition #1
Partition Size 232.88 GB (250,056,015,872 bytes)
But, the recognized partitions are (without System Reserved) .....
Drive C:
Size 271.00 GB (290,984,030,208 bytes)
Drive D:
Size 400.00 MB (419,426,304 bytes)
Drive E:
Size 406.26 GB (436,217,049,088 bytes)
Free Space 332.34 GB (356,843,413,504 bytes
So, windows is trying to boot from Disc 0 Partition 1 (labeled as C) Which is on the HDD, and Disc 1 Partition 1 (the SSD) is missing, apparently.

We need to understand the situation a bit more clearly. Follow Disk Management - Post a Screen Capture Image and let us see it.

After seeing it, we may decide the next steps.
