I had more of these error, and the latest one happened with mIRC uninstalled. Here is the latest dmp file, once again, extension changed to TXT so I could attach it.
Updating the drivers didn't change a thing, but did end up disabling other things (Updating the Bluetooth drivers disabled the touch screen capabilities, no idea why) so I rolled them back.

The removal of System Mechanic worked for me too!!! Thanks. I have a new HP Windows 7 machine and have been getting the APC Index Mismatch blue screens every day. In my case it was using Windows Explorer to look at files. Whenever I looked at 4-5 jpg files using Windows Explorer the system would crash. I read the note above and removed SMP and everything is fine. Thanks again!
Just thought you guys might like to know, i've also removed SM Pro and the issue has gone.
Removed SysMech Pro and funnily enough both the APC_INDEX_MISMATCH and NTFS.sys BSODS have stopped. And for a cherry on top, just go an email from Iolo Tech about it. Supposedly they are working on a solution.

I also was having the apc_index_mismatch BSOD's. It was happening every time I tried to start programs from the Office 2010 Beta, so I thought that might be the problem, but when I uninstalled System Mechanic Pro, everything went back to normal.
Iolo responded that the program may not have been installed correctly and instructed me to install from a new download package, but I'm not sure if I trust SM any more. They wouldn't refund the purchase price, but I don't think the hassle is worth trying another install.
Your product is the cause of the problem, and even though I have contacted you, explaining in detail the error and how it was fixed by removing your product (Yes, I re-downloaded the installation package, uninstalled it by using Revo Uninstaller, reinstalled it) and you keep trying to say it's something I must have done wrong. I have been a Beta tester, and program reviewer for 15+ years. Maybe not for you, but for others. Your software, System Mechanic Pro 9 is a failure with Windows 7. There are dozens of Windows 7 Forums where there are threads and threads of evidence that your product is faulty on Windows 7 machine. I will be contacting my credit card company and opening a dispute to have my purchase price of SM Pro refunded, unless you promptly do so yourself.
I hope the two problems were related, because I think I've solved one. I unplugged my M-Audio Firewire Solo unit, and no BSOD lately. This unit is an external soundcard, and it doesn't have any windows 7 specific drivers yet, it appeared to be working fine, but when I played an online game the RAM usage would gradually go crazy higher than I would have expected, and then I'd get the bsod.

Thought people here might like to know, I got my refund. I was polite, but extremely angry. This is what I sent them after their sending me 3 emails with the "you must have done it wrong".
This is the same generated answer you have given everyone who has had this problem, and it bull. Pure bull.
It was installed properly, 10 times. And uninstalled properly, 10 times. I have already done all the "removal of residual files" routine 10 times. I have both used a professional uninstaller program, ripping out every trace of our useless, system crashing software and then manually searching the registry and not so hidden folders for leftovers. It was then re-downloaded 9 times, then re-installed 9 times.
I'm not a tech newbie, and I truly resent your company knowing there is a major flaw with this software when run on Windows 7 machines but you keep insisting it's the end user screwing things up. Guess again. Your product doesn't work. It crashes systems.
I want my money BACK, and I will go back to using my System Mechanic standard, as Pro is nothing more than a system crashing, file corrupting menace! Do you get it now? Or do I need to use smaller words?
BTW, you should have paid attention when I said I do product and software reviews. You really should have.
