Symantec is standing out quite obviously here.
    Code:
    fffff880`211d9d40 fffff880`04f90628Unable to load image \??\C:\ProgramData\Symantec\Symantec Endpoint Protection\12.1.2100.2093.105\Data\Definitions\BASHDefs\20150814.011_306\BHDrvx64.sys, Win32 error 0n2
*** WARNING: Unable to verify timestamp for BHDrvx64.sys
*** ERROR: Module load completed but symbols could not be loaded for BHDrvx64.sys
     BHDrvx64+0x16e628
Uninstall Symantec and install MS Security Essentials and MalwareBytes.

       Note
Make sure you uncheck the 'Enable Free Trial' box when installing.
